Martin este un singuratic dereglat psihic care trăieşte cu mama lui într-un sumbru complex de locuinţe. El lucrează ca agent de securitate în schimbul de noapte într-un complex de parcare subterană, în aceeaşi măsură sumbru şi prevestitor. Ca să scape de existenţa lui tristă, Martin se pierde în lumea fantezistă a filmului de groază The Human Centipede: First Sequence, imitând îndemânările chirurgicale meticuloase ale talentatului dr. Heiter, ale cărui cunoştinţe avansate despre sistemul gastro-intestinal uman îl inspiră pe Martin să încerce inimaginabilul.
